Transnasal Laryngoscopic Blue Laser Procedure

Otolaryngology

Transnasal Blue Light Laser Procedure is a surgery to treat vocal cord disorders. It is commonly used to remove non-cancerous growths on the vocal cords, such as nodules, polyps, or cysts, as well as other conditions like vascular abnormalities or early-stage laryngeal cancer.

Principles

Transnasal Blue Light Laser Procedure (Blue Laser) is a medical procedure that uses a specific wavelength of blue laser light to treat laryngeal diseases, particularly vocal cord conditions such as nodules, polyps, cysts, or early-stage laryngeal cancer. This surgery is performed through the nostrils, avoiding the need for incisions, thereby reducing patient discomfort and recovery time.

Basic principles of the procedure:

  • Selective absorption of the laser: The blue light laser has high selectivity and is absorbed by specific tissues (such as blood vessels or diseased tissue) while having minimal impact on surrounding healthy tissue. It allows the surgery to precisely remove or destroy the target tissue while preserving the integrity of the surrounding healthy tissue.

  • Minimally invasive approach: The transnasal approach allows doctors to directly access the vocal cords and larynx without requiring complex procedures through the mouth. It reduces the pain and risk of infection associated with surgery.

  • Real-time imaging: Doctors can see the larynx on a screen in real-time using a laryngoscope in conjunction with the laser device. It enables more precise treatment and allows for real-time adjustment of the surgical strategy.

Indications

The following common ENT (ear, nose, and throat) conditions can be treated with Transnasal Blue Light Laser Procedure:

PolypsSoft tissue protrusions are commonly found in the nasal cavity, sinuses, or other mucous surfaces, usually caused by inflammation or allergies.

Reinke’s edema

A type of submucosal vocal cord swelling often caused by smoking, overuse of the voice, or chronic laryngitis.

Ectatic vessels

Abnormal dilation of blood vessels that may appear on the nasal cavity, sinuses, or other mucosal surfaces.

Dysplasia

Abnormal tissue overgrowth or development, which may be benign or precancerous.

Granuloma

Small inflammatory cell clusters that form lumps in the tissue, commonly found on the vocal cords, trachea, or larynx.

Scar tissue

Hardened tissue formed after healing can lead to dysfunction or other complications.

Papilloma

Benign tumours are usually found in the mouth, larynx, or other mucosal surfaces.
LeukoplakiaWhite patches on the mucous membranes of the mouth may be a sign of a precancerous condition.
BiofilmMucous or gel-like biological structures formed by microbial colonies, typically found in the mouth or nasal cavity, can lead to infections or other problems.

Advantages

Blue Laser offers several advantages in treating ENT (ear, nose, and throat) diseases, particularly for specific conditions affecting the vocal cords and larynx.

  1. High Precision:
The blue laser provides exceptionally high precision, enabling accurate targeting and ablation of pathological tissues while causing minimal damage to surrounding healthy tissues. It is especially crucial for surgeries involving the vocal cords and larynx, where precision is paramount.

  2. Minimally Invasive, Quick Recovery:
Because laser surgery typically requires smaller incisions, the procedure is less invasive, with minimal bleeding and reduced risk of infection. Consequently, patients experience a significantly shorter recovery time compared to traditional surgery.

  3. Reduced Pain and Discomfort:
Laser surgery generally causes less pain and discomfort than traditional methods. Patients experience higher postoperative comfort and require fewer pain medications.

  4. Preservation of Function:
The precision of the blue laser allows for better preservation of critical functions, such as voice production and swallowing, which is particularly important for vocal cord surgeries.

  5. No Hospitalization Required:
Many blue laser surgeries can be performed on an outpatient basis, eliminating the need for hospitalization. It not only eases the burden on the healthcare system but also allows patients to recover at home, enhancing their quality of life.

  6. Broad Applicability:
Blue laser surgery can treat a wide range of ENT conditions, including but not limited to vocal cord nodules, polyps, cysts, and even some types of early-stage laryngeal cancer.

Blue Laser provides an efficient, minimally invasive treatment option for ENT diseases, particularly those requiring precise operations and preservation of vital functions in the larynx and vocal cords.

Head and Neck Cancer

Clinical OncologyOtolaryngologyGeneral Surgery
Head and neck cancer is a collective term for cancers that occur in various upper respiratory tract tissues. Depending on the site of occurrence, it can be divided into oral cancer, nasopharyngeal cancer, oropharyngeal cancer, hypopharyngeal cancer, and laryngeal cancer. The most common tumour type in head and neck cancer is squamous cell carcinoma, which originates from squamous cells on the mucosal surface of the head and neck. According to medical research, some head and neck cancers are associated with HPV (human papillomavirus).

Nasopharyngeal Cancer

Nasopharyngeal cancer is one of the top ten cancers in Hong Kong that cannot be ignored. There are 600 to 800 cases of nasopharyngeal cancer in the city every year. Unlike many other cancers, nasopharyngeal cancer tends to occur at a relatively early age, with the peak incidence between 40 and 60 years old, considered the prime of one's life. The incidence rate is almost three times higher in males than in females. Compared to Western countries, nasopharyngeal cancer is more commonly found in Southern China.

Oral Cancer

Clinical OncologyOtolaryngology
The "oral" region comprises multiple sites, including the lips, tongue, floor of the mouth, hard palate, and gums, as well as the oral mucosa, buccal mucosa, and salivary glands. Any malignant tumour growing in these locations is collectively referred to as oral cancer, which is also one of the types of head and neck cancer.
